Patient portal: access to your documents

The Stichting Med Partners patient portal provides access to available documents, questionnaires and information after your visit.

Activate your account

  1. Open the official patient portal using the instructions provided by the practice. Use an up-to-date browser.
  2. If you do not yet have an active account, select account aanmaken (“create account”) and use the email address given when booking.
  3. Check the activation email, including Spam, Junk, Ongewenste e-mail, Reclame and Promotions.
  4. If you have forgotten your password, select wachtwoord vergeten and follow the email instructions.
  5. If a code is requested, codegenerator instellen means “set up a code generator”, an additional security feature. Contact the practice if you changed phones, removed the app or cannot obtain a code.

After signing in

Look for documenten (documents), afspraken (appointments), vragenlijsten (questionnaires), dossier (record) or verslagen (reports). Available sections depend on your account and visit.

Never share passwords or codes

Staff can check account activation and the recorded email address. They never ask for your password or personal login codes. Do not send these by email, text or WhatsApp, or share them with other people.

Cannot sign in?

  • Check that the account is active and you are using the booking email address.
  • Check your spam folder.
  • Identify whether the issue is a password, security code or missing email.
  • Search your inbox for Stichting MED PARTNERS, uwvragenlijst, ZorgAdmin, E-health or patiëntportaal.

Ask administration to check activation and your registered email address if needed. The normal practice process asks for your name, date of birth, booking email address, telephone number and appointment date to identify the request.
