High blood pressure and cardiovascular risk
Raised blood pressure needs repeat measurements and assessment of overall cardiovascular risk. Dr Krzysztof Chyrek, physician (Arts), BIG registration 89917610801, discusses measurements, results and next steps. This consultation does not replace ongoing care from your huisarts.
Stichting Med Partners does not provide emergency care. Call 112 if life is in danger and your huisarts or huisartsenpost for other urgent symptoms. Do not wait for an online booking.
Why is one reading insufficient?
Blood pressure changes during the day. Persistently raised readings can increase risks to the heart, blood vessels and kidneys even without symptoms. Assessment requires correctly repeated measurements, for example at home under an agreed schedule or over 24 hours. Home and clinic readings are interpreted differently. Do not use one universal threshold yourself or change medication after a single reading.
Measuring at home
- Avoid exertion, smoking and caffeine for about half an hour beforehand. Sit quietly before measuring; Dutch patient guidance specifies at least five minutes.
- Keep feet flat and legs uncrossed, with your back and arm supported. Put a correctly sized cuff on the bare upper arm at heart level. Use a reliable upper-arm monitor, the agreed arm and the manufacturer's instructions.
- Do not talk or move. Take two readings about two minutes apart.
- Record both readings, pulse, date and time. SYS is systolic pressure, the upper number; DIA is diastolic pressure, the lower number. Note symptoms, stress, caffeine and medication without changing doses yourself.
- Measure at similar times, usually morning and evening. The source describes seven days, twice per session. Morning readings are before breakfast and with medication timing agreed with your doctor, not immediately after waking. Dutch guidance describes evening readings two hours after dinner. Individual instructions take priority.
Bring an incomplete diary too. Do not change medication timing or doses yourself. Do not wait to complete a diary when symptoms are urgent.
Overall risk matters
The doctor also considers age, smoking, cholesterol, diabetes, kidney function, previous cardiovascular disease and lifestyle. SCORE2 or SCORE2-OP may estimate ten-year risk for some people but is not suitable for everyone. Existing conditions can determine risk category. Region of origin may influence the choice of risk chart. An estimate is not a prediction of certainty, and low ten-year risk does not mean no longer-term risk.
What can you discuss?
Stopping smoking, suitable activity, diet, salt, alcohol and weight. Tests, monitoring and medication depend on readings and health; markedly high blood pressure still needs assessment even with low calculated risk. Discuss benefits, adverse effects and preferences. Do not start or stop medication without agreement.
Preparation
Bring your diary, medication and doses, supplements, non-prescription medicines, results and earlier records. Note questions, symptoms and difficulties taking medication. A consultation may review history and records and plan investigation, monitoring or referral. Booking does not automatically include ultrasound or a test package.
When to seek help
Call 112
For chest pain, sudden severe pain between the shoulder blades, severe breathlessness, or sudden limb weakness, facial drooping or speech difficulty. Do not wait for another reading.
Call your huisarts or huisartsenpost immediately
- SYS above 180 mmHg with headache, nausea, impaired vision or dizziness.
- SYS above 200 mmHg, higher than usual and remaining so for the rest of the day.
- A much higher pressure than usual with these symptoms.
- SYS below 90 mmHg and much lower than usual.
- DIA above 120 mmHg, higher than usual and remaining so for the rest of the day.
Do not delay contact. For SYS between 180 and 200, higher than usual without these urgent situations, the source advises arranging GP contact within a week. Also discuss a week's average above your agreed target.
Common questions and consultation
Hypertension often causes no symptoms. Not everyone needs medication; the decision is individual. Keep your huisarts for ongoing and urgent care. Private consultations in Polish take place at Euromarkt 34, 2408 BL Alphen aan den Rijn. The booking page lists ages 18–70; check uncertain cases with administration.
